I've forgotten to include my laboratory request form with my sample.

Each sample kit will come with a laboratory request form. 

This is a piece of pink folded paper containing your details and the details of your test, and is used at the laboratory to identify your sample and the tests that are needed.

It's important that you complete this form and include this within your sample kit when you post this back to us. 

If you have your sample taken at a partner clinic or by a home nurse, they will help you complete this form.
