When you complete your test, you will receive an email from us that says we need some more information before we process your test. This is your invitation to complete the Supporting Information questionnaire, if you have not already done so.
You can find the Supporting Information questionnaire, along with the Lifestyle Profile and the Health and Medical History questionnaire, in your customer portal at my.medichecks.com.
Log in and you'll see your recent tests on the main dashboard under 'My Pending Tests'. To access your Supporting Information questionnaire, click the test name, and complete the questions that appear on the next page.
Please note: this questionnaire will lock once your test results are with the reporting doctor for comment, and you will not be able to make changes after that point.
