I have received an email with a link to my Supporting Information questionnaire, but I cannot open it.

If you’re struggling to open the email link to your Supporting Information questionnaire – don’t worry! This can be accessed via your account instead.

Simply log into my.medichecks.com with your username and password. On the main page, you’ll see your pending tests. Click on your test, and the Supporting Information questionnaire will load. 

You do not have to complete this, but it can provide our reporting doctors with context as to why you’re taking your test.

This questionnaire will lock when your results are with the reporting doctor for comment, so we recommend that you complete this on the same day your sample was taken.
